If your deductible resets every January, your plan doesn't cover the procedure you need, or you simply want to skip the insurance back-and-forth, paying directly is often the faster and cheaper route. Modal Pain Management publishes its self-pay prices in full so you can decide before you ever sit down in the office.
Every price below is all-inclusive. It covers Dr. Movshis's physician fee, the ultrasound or fluoroscopic guidance used during the procedure, and the medication — charged per session as the procedure is typically performed. There is no separate facility fee. If your case needs something different, that's confirmed in writing at your consultation before anything is scheduled.

How much does a pain management visit cost without insurance in NYC?
At Modal Pain Management, a self-pay new-patient consultation with Dr. Alex Movshis is $250 and a follow-up visit is $150. These are flat, all-inclusive prices — there is no separate facility or paperwork fee. Individual procedures are priced separately and listed in full on this page.
Are your self-pay prices all-inclusive?
Yes. Each listed price covers the physician fee, the imaging guidance used (ultrasound or fluoroscopy), and the medication, charged per session as the procedure is typically performed. There is no separate facility fee. Your exact plan is confirmed at your consultation before anything is scheduled.
Can I use my HSA or FSA?
Yes — HSA and FSA cards are accepted for medically necessary visits and procedures. Some wellness services, such as NAD+ or elective IV infusions, may not be HSA/FSA-eligible unless they are medically indicated, so check with your plan administrator first. We provide an itemized receipt for your records.
Do you offer payment plans? Can I pay over time?
Yes. Modal Pain Management offers "pay over time" plans through a healthcare financing partner, including 0%-interest options on shorter terms and low-interest plans for larger amounts. How much does an epidural, a cortisone shot, or RFA cost without insurance?
Self-pay prices at Modal Pain Management: an epidural steroid injection is $1,000 for the lower back (lumbar/sacral) and $1,500 for the neck or upper back (cervical/thoracic). A cortisone shot for a joint or bursa is $700. Radiofrequency ablation (RFA) is $1,500 for the lower back and $2,000 for the neck. Facet (medial branch) nerve blocks are $1,000 (lower back) and $1,500 (neck). Every price is flat, all-inclusive, and charged per session.
Why pay self-pay if I have insurance?

Can I get a superbill to submit to my insurance for reimbursement?
Yes. For any self-pay procedure, we provide an itemized superbill with the CPT procedure codes and ICD-10 diagnosis codes your insurer needs. Many PPO plans reimburse a portion of out-of-network costs after your out-of-network deductible is met. You submit the superbill directly to your carrier.
